⚠️ TEMPLATE — ATTORNEY REVIEW REQUIRED BEFORE PUBLISHING. Bracketed fields [LIKE THIS] must be completed before this is finalized.

1. Educational Purpose Only

PCOS Nav (the "App") is an educational and informational tool. It is not a medical device, and it does not provide medical advice, diagnosis, or treatment. The App is operated by KANDIKILLS LLC. The content, features, and outputs of the App are intended to help you organize information and prepare for conversations with qualified healthcare professionals — not to replace them.

2. Not a Diagnosis

Polycystic Ovary Syndrome (PCOS) is a medical condition that can only be diagnosed by a qualified healthcare professional. The App's phenotype quiz is based on your self-reported answers and produces an informational pattern only — not a clinical diagnosis. One of the recognized diagnostic criteria requires an ultrasound that cannot be self-reported. Any pattern, type, or category the App suggests is a starting point for discussion with your clinician, not a determination that you do or do not have PCOS or any subtype of it.

3. Lab Information Is Not a Clinical Interpretation

The App allows you to enter laboratory values for your own reference and to compare them against general informational ranges. These ranges may differ from the standard reference ranges used by your laboratory or clinician, and reasonable medical professionals may interpret the same values differently. The App's lab feature is not a diagnostic service and does not interpret your results in a clinical sense. Always review your actual lab results with the healthcare provider who ordered them. Do not start, stop, or change any medication, supplement, or treatment based on what the App displays.

4. Supplements Are Not Treatments

Any information the App provides about dietary supplements is for general educational purposes. Dietary supplements are not intended to diagnose, treat, cure, or prevent any disease, and statements about them have not been evaluated by the U.S. Food and Drug Administration. Supplements can interact with medications and may not be appropriate for everyone, including people who are pregnant, nursing, or managing other health conditions. Consult a qualified healthcare professional before beginning any supplement.

5. No Provider–Patient Relationship

Using the App does not create a doctor–patient, therapist–client, or any other professional relationship between you and KANDIKILLS LLC or anyone associated with it. The App does not know your full medical history and cannot account for your individual circumstances.

6. Advocacy Scripts

The App may generate suggested language to help you raise topics with your healthcare provider. This language is a communication aid only. It is not a recommendation that you receive any specific test, medication, or treatment, and your provider remains the appropriate person to decide what is right for you.

7. Emergencies

The App is not for emergencies. If you think you may have a medical emergency, call your doctor or your local emergency number immediately. Do not rely on the App for urgent or emergency medical needs.

8. Assumption of Responsibility

You are responsible for your own health decisions. To the fullest extent permitted by law, you assume all responsibility and risk for your use of the App and any decisions you make based on it. By using the App, you acknowledge that you have read and understood this disclaimer.

9. Contact
